- Up-To-Date CV.
- Copy of Postgraduate Diploma in Sports & Exercise Medicine (SEM) Certificate (ie. Diploma / Masters in SEM - Sports Doctors, DIMC - Crowd Doctors).
- Copy Full GMC Certificate.
- Copy REMO or ATLS + ALS Certificate (Sports Doctors)
- Professional Indemnity Cover Note - MPS/MDU
- UK Hepatitis B Lab Report.
- Copy of Enhanced Disclosure (CRB) Certificate - valid within a year.
- Copy of Passport and two passport photographs.
- Copy of JCPTGP/CCT Certificate - For GPs with Sports Medicine qualifications.
